If you have questions about any of the medical information requested, please call 207-778-7064 or e-mail Karin Krzal at karin.krzal@maine.edu.

While most of the forms in each packet are required by the Department of Athletics, we have also included a physical evaluation and interval health history form that is used by the Student Health Center. The physical form should be used if you will be having a sports physical done by your own physician. The interval history form should be used if you have had a physical done at the Student Health Center within the last year.  

Please be reminded that a NEW PHYSICAL IS REQUIRED EACH YEAR, unless you have had a physical done at UMF in the past.  You will not be permitted to tryout, practice or compete until UMF has a current physical on file.
